How to add a Form (Questionnaire) to the Email?

1. Go to the Form Builder, open up the From that you want to add to your email, click on the 'Form Merge Tag' and Copy it.

2. Open up the email where you want to add the form, click on the Button block to edit it and paste the Form Merge tag in the button and click submit.
